California’s Health Homes Program: Lessons Learned and Promising Practices

The Health Homes Program (HHP) is a Medi-Cal initiative that helps manage and coordinate care for enrollees with certain chronic health and/or mental health conditions who have high health care needs or who are experiencing chronic homelessness. The California Department of Health Care Services (DHCS) oversees the administration of the HHP; and 17 Medi-Cal managed care plans (MCPs) are operating the program in 12 counties. This paper is based on key lessons learned and promising practices identified throughout learning collaborative sessions. Harbage Consulting supports HHP implementation by planning the learning collaborative sessions, developing educational materials for Medi-Cal managed care plans, and conducting on-the-ground outreach to stakeholders.
