Innovative Care Coordination Programs for California’s Medi-Cal Beneficiaries: The Health Homes Program and the Whole Person Care Pilots

The State of California is operating two innovative programs that provide care management and care coordination services to Medi-Cal beneficiaries with high health care needs – the Health Homes Program (HHP) and the Whole Person Care Pilot Program (WPC). These programs represent similar, but structurally different, efforts to improve care coordination for beneficiaries across the health care system and address their social determinants of health. This paper summarizes the key objectives and features of these programs, highlights their similarities and differences, explains the requirements for Medi-Cal beneficiaries who qualify for both programs, and suggests some ways the programs can coordinate to avoid duplication of services.
